Blood Transfusion Errors
The objective of this document is to highlight hospital errors such as mislabeled specimens and the importance of patient identification and the accountability aspect of these errors. An accurately labeled and drawn sample for pre-transfusion and compatibility testing is the preliminary steps in ensuring patients obtain a secure transfusion. Nothing is more essential to protected and efficient patient care than ensuring the caregiver is performing the accurate procedure on the intended patient.
